In order for UCI to obtain your images from another non-local facility, you must complete the Medical Records Release form.

Did you know your images and reports are now available to you on your Patient Portal?  Please contact our facility if you need assistance logging in.  All of our providers also have access to these images in their office through our PACS.

If you are a patient need a disc/report, you can make your request in-person at either our Oaks or Suntree locations at the Front Desk.  While discs are typically available 24 hours after your exam, reports will not be available until 3 business days after your exam.  Please sign in on the iPad as a “Pick Up” and be prepared with your photo ID.
