In order for UCI to obtain your images from another non-local facility, you must complete the Medical Records Release form.
Did you know your images and reports are now available to you on your Patient Portal? All of our providers also have access to these images in their office through our PACS.
If you are a patient and need a disc/report, please make your request in-person at either our Oaks or Suntree locations at the Front Desk. While discs are typically available 24 hours after your exam, reports will not be available until 5 business days after your exam. Please sign in on the iPad as a “Pick Up” and be prepared with your photo ID.
Out of State/Non-Local disc requests ONLY: please complete the Images/Report Request form or fax your request to 321-726-3846.
