Medical Records

In order for us to release your images to another facility, you must
complete the Medical Records Release form. You may fax it to
321-726-3846 or drop it off at one of our facilities.

University Center Imaging retains exams electronically. Some physicians
have access to these images in their office through our PACS.

If you need a CD of your images, please contact the Medical Records
Department at 321-726-3816 or , 24 hours prior to pick up.
